Celebration of Life Sponsorship
Dear Friends,
We are pleased to be recognizing our 25th Anniversary of the ART Fertility Program of Alabama with a program that will allow five eligible couples the opportunity to receive an IVF cycle at no charge to them.
We are excited you are applying for the “Celebration of Life” sponsorship. Applications will be accepted via USPS and should be postmarked from Father’s Day, June 20, 2010 through Labor Day, September 6, 2010 with hopes that those couples chosen will begin cycles in early 2011.
Below is the list of criteria that eligible couples must meet in order to be accepted and possibly chosen to receive the sponsorship.
- IVF is the next medically indicated therapy
- Has completed no more than one previous IVF cycle
- Residents of State of Alabama
- No children from either partner
- Female < age 35, normal ovaries* and uterus**; male < age 45, adequate sperm for IVF; both non-smoking.
- Insurance that covers pregnancy but not IVF
- Medically able to undergo treatment and pregnancy
- Income of both partners less than $75,000 annually (tax return required)
- Recommended by OB/GYN and records must be submitted
- Submit written essay of 500 words or less describing why you should receive the cycle.
Five couples who meet the criteria and are located in areas served by our offices (two couples from Birmingham area, one from Huntsville area, one from Montgomery area and one from Tuscaloosa area) will be chosen by a volunteer team of community leaders based on the essay written by the couple.

Couples will be chosen and contacted by Thanksgiving, November 25, 2010 and initial office visits will subsequently be scheduled with one of our physicians to begin the IVF process.
